Electronic claims:   Electronic claims are sent using 837 5010 format. Each loop, each line, each segment requires a specific set of data to be sent electronically. The specifications get even more stringent when sending electronic secondary claims.

Hardcopy Claims (HCFA) Medicare:  Okay, here's the crazy part. The instructions for creating a HCFA form for Medicare are different from the instructions that you would use for commercial carriers. We have included a copy of the Medicare guidelines for creating a hard copy claim.
