Problem:  When trying to use a Treatment Plan, get "Invalid subscript reference"

Solution:  Recreate the offending plan or delete and re-add the rows in the plan.

1. With a patient in the schedule selected, click the Treatment Plan button
2. Click the Search icon next to Treatment Code
3. Click the plan and click the Edit icon
4. Delete all of the rows and add them again.
5. Click Save and Close
