Diagnosis 1-12

Question:  In submitting a claim to any insurance how many diagnoses are accepted on a claim?  

Answer:  The billing program will allow you to submit 12, but some carriers only look at the first one or the first 4. 

Electronically, the diagnosis codes are submitted in the HI segment.  For HCFA, the diagnosis codes are reported in box 21.
